Exam and plan
Your dentist examines you, takes X-rays if needed and explains what they found. You leave with a written plan and estimate.

Before you come in
What do I need on the day?
Your insurance card if you have one, a photo ID and a list of your medicines. If you have recent dental X-rays, bring them or ask your last office to send them.
Do I give my medical history on this website?
No. The request form takes no medical details. Your history goes on the forms in our secure portal.
Can you get my records from my last dentist?
Yes. Give us the name and number of your previous office and sign a release, and we will ask for your records before your first visit.
